Contact Form "Select Level" not visible.

   Hello, I've got an issue I need some help with. For whatever reason I can't the contact form to display a select dropdown box. You can view my page here.... http://www.churchvideos.net/get-started/ The "State" field is the selectable option. What am I missing here?

   Hi selfmadenothing,

   Replace the following file on your server with the .zip file attached to this post (don't forget to unzip it).

   wp-content/themes/method/lib/scripts/custom.js

   Then clear your browser and refresh your browser.

   Let us know if this works for you.
